What to Expect
When you contact us for an appointment, we will ask for the following information:
• Full name including any middle initial • Address • Home phone number • Work phone number • Date of birth • Social Security number • Which clinic location you'd prefer • Primary care and/or referring provider's name • Your primary and secondary insurance information • What symptoms you are having
We will offer you the next available consultation appointment at your preferred clinic location. We will automatically request your medical information from your primary care provider prior to your visit. If you have any relevant X-rays, please let us know when you schedule so that we may request the films.
Forms:
When you check in on the day of your appointment, you will have forms to complete, and we will give you an introductory letter. The forms include:
• A brief list of your current symptoms • A list of current medications, allergies and surgeries
•An optional form to authorize us to talk with any family members or friends about your medical information, or release medical records to another physician. • A registration form for you to verify the information you have already given us. The only additional information is a pharmacy name and an emergency contact. • A financial responsibility form which allows us to bill your insurance company. • Notice of Privacy Policies, which provides information on how we handle protected health information.
Insurance
We will also make a copy of your insurance card(s) for our files. If you do not have an insurance card(s), you will need to complete a waiver form that assigns all financial responsibility to you. Any co-pay would be collected at the same time. If you fail to bring in your co-payment you may be asked to reschedule your appointment.
Examination
The provider will bring you back to his/her office to discuss your symptoms and relevant medical information. If a physical exam is needed, you will go to an exam room. After the exam, the provider will meet with you back in his/her office to discuss any tests or procedures that he/she recommends. The provider will walk you back to the front desk for any scheduling.
